Assessments
This is a assessment by the therapist to determine if the issues
outlined in your Evaluation can lead to a diagnosis that your impairment
“substantially limits one or more major life activities” as term is used
in the American with Disabilities Act (ADA).
You will need to submit a new Evaluation form with any additional information you feel may be helpful in explaining your condition.
There is no guarantee that your assessment will result in a diagnosis
that you are impaired with in meaning of the ADA, as that is solely
within the discretion of the therapist.
The therapist will also assess whether you would benefit from having a PSD to assist you in
dealing with your disability. Should such an assessment be made, the training of the psychiatric
service dog will be your sole responsibility. The therapist cannot and will not provide any
direction or instruction in that regard.
A diagnosis of an impairment is intended for your use only and for the sole purpose of establishing your need for a self- trained Psychiatric Service Dog.
It is not intended to be used as a prescription for any third party trainers, nor may it be used to claim any additional rights or benefits under the Americans with Disabilities Act.
The fee for assessment is $150.

These letters will be valid for a period of one year. Renewals will be
available at a discounted fee of $95.
